Please complete this form and submit.  You must then send a check for the correct amount to the address above.  The Membership Secretary, who will forward all satisfactorily verified applications to the Executive Committee, will verify information furnished on the membership application/submittal.  Each application/submittal will be reviewed at the following month's meeting of the Executive Committee and either accepted or rejected.  Individuals will be notified of the action taken on each application.
